Vacation/Personal Day Policy
CSMI employees will be allotted Vacation days on January 1 based on the schedule below:
One personal day will be allotted on January 1, April 1, July 1 and October 1 after 90 days of full-time employment.

Upon termination of employment you will be paid for any unused vacation days you have available.
GIVING BACK
CSMI employees will be allotted up to 8 hours of additional vacation time to be used exclusively for the CSMI Giving Back program. In order to receive this additional vacation allotment you will need to submit your approved Giving Back cause form to Payroll. 
To be eligible for vacation/personal days, an employee must be full-time and meet the following requirements:
- Employee must be scheduled to work at least 30 hours per week.
- Employee must not be a temporary employee hired for a definite duration of time, such as a co-op student.
- Employee must have completed 90 days of continuous employment.
RATE OF PAY FOR VACATION/PERSONAL TIME
Vacation/personal time will be paid when vacation/personal time is taken. The rate for computing the amount of pay is the base rate that is in effect when the vacation/personal time is taken or on the last day worked.
UNUSED VACATION
Vacation/personal time off must be scheduled with your Supervisor or Department Manager at least 24 hours in advance.
Carryover: Employees will be allowed to carry-over up to 10 vacation/personal days into the next year.
Cash-in: Salaried employees will not be allowed to cash-in vacation/personal days. Hourly employees may cash-in up to 10 vacation/personal days. The cash-in period will be between December 1 and December 31. Management reserves the right to require employees to take all their vacation/personal days or to carryover up to 10 days before cashing any in.
Forfeitures: Any vacation/personal days that are not used, carried over, or cashed in will be forfeited.
TIME OF VACATION/PERSONAL DAYS
Vacation/personal days can be taken January 1 through December 31. Requests for vacation/personal days should be made two weeks in advance; however, whenever business conditions permit, with as much notice as possible, vacation/personal days will be granted when requested. Vacation/personal days may be taken in two hour increments. Employees may request vacation/personal days on Saturday and Sunday if they are normally scheduled to work these days.
A minimum of five (5) consecutive scheduled vacation/personal days must be taken off at least once during each calendar year for team members receiving 10 or more vacation/personal days during that year.
MEDICAL INSURANCE
Employees are eligible for medical insurance on the first day of the month following 30 days of full-time employment. The information below is not conclusive nor does it take precedence over any information provided in the medical plan document. Employees can choose from the two Conservative Plans as described below.

Various Adult Routine and Preventive Child Care examinations are paid in-network at 100% of allowed charges up to the maximum allowable of $500, not subject to your deductible or any applicable co-payments.

50% for a non-preferred brand
name drug up to $250/Rx
DENTAL INSURANCE
Employees are eligible for dental insurance on the first day of the month following 30 days of full-time employment. The information below is not conclusive nor does it take precedence over any information provided in the dental insurance plan document.
Summary of Benefits
Deductible: $50.00 for benefits received in Coverage B, Coverage C, with a maximum of $150.00 per family per benefit period. There is no deductible on Coverage A. Annual Maximum Payment: $1,000.00 Per Person Per Calendar Year
CAFETERIA PLAN
A cafeteria plan exists enabling employees to pay insurance premiums with pre-tax dollars. In addition, a medical savings account and childcare account are available within the cafeteria plan.
EMPLOYEE STOCK OWNERSHIP PLAN (ESOP)
The E.S.O.P is the plan governing the employee stock ownership trust. The trust was set up to benefit team members by allowing them to hold stock in Central States Manufacturing. Employees receive stock allocations based on a percentage of wages in years which they qualify based on the plan document. Vesting is over a period of six years in which the participant works at least 1000 hours.
401(k)
A 401(k) plan is in place for those wishing to participate. The company will match 100% of the first 4% of wages the employee contributes to 401(k). An employee is eligible to enter the plan at the beginning of each quarter after the 90-day probationary period. Vesting in the company contribution is over a period of six years in which the participant works at least 1,000 hours. The employee is always 100% vested in his own contributions
EDUCATION ASSISTANCE PROGRAM
At CSMI we recognize that our employees skills and knowledge are critical to our success. To support the development and growth of our employees we offer an Educational Assistance Program, which provides reimbursement for approved educational expenses.
